Glaucoma cannot currently be cured, but it can almost always be controlled. With appropriate treatment to usually pressure-lowering eye drops, and in some cases laser or surgery to most patients retain useful vision for life. The critical point is that vision lost to glaucoma does not come back. Early detection is not just convenient; it is the difference between preserving your sight and managing its gradual loss.
In primary open-angle glaucoma, the most common type, raised intraocular pressure slowly damages the optic nerve. However, some people develop glaucoma with normal pressure to called normal-tension glaucoma to where the optic nerve appears vulnerable even without elevated IOP. Risk factors include age over 40, a first-degree family member with glaucoma, high myopia, diabetes, long-term corticosteroid use (eye drops or tablets) and certain anatomical features of the drainage angle. A glaucoma assessment here includes IOP measurement, optic disc examination through dilated pupils, automated visual field testing (perimetry) to map peripheral vision, and OCT of the nerve fibre layer to detect early structural changes. Some patients notice redness, mild stinging or, with prostaglandin drops, gradual darkening of the iris or eyelashes. Most people tolerate drops well with no noticeable systemic effects. If a particular drop causes problems, alternatives exist. What is important is not to stop glaucoma medication without discussing it with your doctor to uncontrolled pressure is silent and the damage it causes accumulates without warning.
